Attending Physician Statements and
Disability Forms
Insurance companies and employers often require
physicians to complete forms for leave of absence from
work or for disability. When these forms are required,
please forward them to our office as soon as you receive
them. Due to the complexity of the form, the information
required and physician signatures, please allow the
office 7-10 working days to complete these forms
Referrals
Getting a referral to see a specialist is often required
by insurance companies. Before scheduling your appointment,
check with your insurance company to see if a referral is
required. If so, you will need to contact your primary care
physician to get a referral. At the time of your appointment,
if a referral is required and has not been received in our
office, you will be asked to sign a Missing Referral Form
before being treated by your physician. This states that
if your insurance does not pay because of a missing referral,
the bill becomes your responsibility.
Billing & Insurance
Current patient information is required to ensure accurate
billing to your insurance company. Please let us know if
your address, phone number or insurance has changed. Your
insurance company will be billed for all charges that you
incur. Deductibles and co-pays are your responsibility and
should be paid at the time of your visit. You will receive
a statement letting you know of any remaining balance that
is your responsibility after your insurance has paid.
